Appointments are scheduled based on availability. You agree to provide reasonable notice for cancellations or rescheduling. Failure to be available at the scheduled time may result in service fees or forfeiture of payment.
Payment may be required prior to service. Fees cover specimen collection and administrative services and may exclude laboratory testing fees unless otherwise stated.
Refunds, if any, are determined at our discretion and may depend on service completion, cancellation timing, or laboratory policies.
Your use of the services is subject to our HIPAA Authorization and Privacy Policy, which govern how your health and personal information is collected, used, and disclosed.
You acknowledge that specimen collection involves minimal risks such as discomfort, bruising, dizziness, or infection. By using our services, you voluntarily assume these risks.
To the maximum extent permitted by law, iCare shall not be liable for any indirect, incidental, special, or consequential damages arising out of or related to your use of the website or services.
Our total liability for any claim shall not exceed the amount paid by you for the applicable service.
You agree to indemnify and hold harmless iCare, its officers, employees, contractors, and agents from any claims, damages, losses, or expenses arising from your misuse of the services or violation of these Terms.
